Megan Khung's death: Police officer resigned & supervisor given financial penalty over handling of case

They had not escalated the case to the regular case review sessions with their supervisors.

The duty investigation officer (IO) who first handled the initial report of four-year-old Megan Khung has since resigned from the force.

Megan died on Feb. 22, 2020, at the hands of her mother and her mother's boyfriend after being subjected to months of abuse, during which social workers and her grandmother were looking for her to no avail.

Both the officer and her supervisor, the duty officer-in-charge (OC), were investigated for a disciplinary offence of neglect of duty in relation to Khung's case.

The supervisor was given a financial penalty and is still serving in the force, the Singapore Police Force (SPF) said in response to Mothership's queries.

Report was assessed to be "low safety concern"

SPF confirmed with Mothership that Megan’s grandmother had lodged a police report in January 2020, stating that she had not been in contact with Foo for some time and that Foo had blocked her mobile number.

There was "no indication of child abuse" in the report, SPF said.

SPF said the report was assessed as a case of child discipline with "low safety concern" for several reasons.

First, the discovery of bruises on Megan was made in March 2019, which was assessed by the school and Beyond Social Services (BSS) to be a "case of excessive discipline" by Foo.

A temporary care plan was put in place for Megan to reside with her grandmother.

Following which, there were no other reports of suspected abuse between Mar. 19, 2019 and Jan. 17, 2020 ( the day of the report).

Foo had also video-called the grandmother, and the grandmother had seen Megan, during which she had "looked well", SPF said.

Following the report, the IO conducted screenings to find out more about the parties involved in the case and attempted to contact Foo over the next two weeks.

"Due to SPF’s extensive involvement in the COVID-19 operations, many frontline officers, including the IO, were deployed for COVID-19 duties. The IO did not follow up thereafter," SPF said.

Lapses in handling Megan's case

According to SPF, the IO and her supervisor "should have followed established police procedures" to escalate the case to the regular case review sessions with their supervisors, for monitoring and guidance.

For the subsequent police reports concerning Megan, procedure was followed and the reports were referred to the regular case review sessions, police said.

Megan’s grandmother and her biological father both lodged separate police reports on Jul. 20, 2020, about half a year later after the initial report.

Foo was located shortly after, and both she and her boyfriend, Brian Wong, were arrested in connection with Megan's murder.

Following the discovery of Megan's death, the IO and her supervisor were both investigated for a "disciplinary offence of neglect of duty", SPF said.

The IO has since resigned, while a financial penalty was imposed on the supervisor, who is currently still in the SPF.
